Low Back Pain: The Complete Columbia, MO Guide to Lumbar Spine Issues

August 22, 20258 min readDerek Parker

The Number One Reason People Miss Work -- And How to Fix It

Low back pain has become increasingly problematic over the past few decades, placing significant burdens on healthcare systems and social systems worldwide. It is one of the most common conditions physical therapists treat, and at SoftWave By MoloTherapy in Columbia, MO, it is the single most frequent complaint that walks through our door.

Here is the frustrating reality: while 80 to 90% of first-time low back pain episodes resolve within six weeks, the small percentage of people who develop chronic low back pain account for 75 to 90% of all costs associated with this condition. That chronic population often experiences reduced quality of life, poor overall health, and mountains of medical expenses.

The Lumbar Spine: Built for Power, Prone to Problems

Your lumbar spine consists of five vertebrae that increase in size from L1 to L5 to accommodate progressively increasing loads. The lumbar vertebral bodies are the largest in the spine, with relatively flat surfaces designed for load transfer.

The lumbar discs are approximately cylindrical, with their shape determined by the integrity of the annulus fibrosus -- the tough outer ring of layered collagen. These layers alternate in direction at about 65-degree angles, which is vital for resisting the twisting forces that the lumbar spine encounters daily.

The posterior portions of the lumbar discs are thinner and more tightly packed than the anterior portions. Because we spend so much of our lives in flexion -- bending forward, sitting, rounding our backs -- the posterior disc takes a beating. Fatigue damage accumulates in the back part of the disc, making it a common site of injury.

The lumbar disc is not a passive spacer. It is a dynamic, osmotic system that holds vertebrae together while simultaneously pushing them apart. When it fails, the consequences affect everything.

Common Lumbar Spine Conditions

  • Disc herniation -- when the nucleus pulposus pushes through the annulus fibrosus and compresses a nerve root. Symptoms typically include back pain with leg pain, numbness, or weakness.
  • Facet joint syndrome -- the lumbar facet joints are vertical and J-shaped, which restricts rotation and resists forward sliding. When they become arthritic or inflamed, they produce deep, achy pain that worsens with extension and rotation.
  • Spinal stenosis -- narrowing of the spinal canal, usually from a combination of disc bulging, facet joint enlargement, and ligament thickening. This is more common in people over 60 and produces leg pain with walking that improves with sitting.
  • Degenerative disc disease -- as we age, the water content of the nucleus pulposus decreases (from about 80% at birth to 68% in the elderly), and the disc loses its ability to absorb shock and distribute forces evenly.
  • Muscle and ligament strain -- the paraspinal muscles and spinal ligaments are common sources of acute low back pain, especially after sudden movements or heavy lifting.

The Sacrolumbar Junction

The L5-S1 junction -- where the lumbar spine meets the sacrum -- is the most stressed segment in the entire spine. It sits at the point where the mobile vertebral column transitions to the fixed pelvis, and the forces here are enormous.

In some people, the last lumbar vertebra fuses to the sacrum (sacralization of L5), while in others, the first sacral segment becomes mobile (lumbarization of S1). These anatomical variations can affect biomechanics in ways that are not always predictable.
