What Cervicogenic Headache Is
A cervicogenic headache is a headache that actually comes from your neck β the top few joints of the cervical spine refer pain up into the head. It's usually one-sided, starts at the back of the neck or base of the skull and spreads to the forehead or temple, and is brought on or eased by neck positions and movement. Treat the neck, and the headache settles.
- Primary β Neck-driven headache β one-sided head pain linked to neck position, stiffness, and movement, often with a stiff, tender upper neck. This protocol is for it.
- Secondary β Other headache types β migraine, tension-type, and others behave differently and are managed differently; if it doesn't track with your neck, it may not be cervicogenic.
Involved tissues: The upper cervical joints (the top three segments) and their muscles, the deep neck stabilizers, and the tight suboccipital muscles at the base of the skull.
Two pillars for cervicogenic headache: first, restore upper-neck mobility and calm the tight suboccipital muscles that refer pain into the head; second, build the deep neck stabilizers and fix the forward-head posture that keeps overloading the upper neck. Free the top of the neck, then control it.
It's the neck, so treat the neck
The pain is in your head, but the driver is in your neck β specifically the top three joints and the small suboccipital muscles at the base of your skull, which refer pain up and over the head. Two things keep it going: stiffness and tension in that upper neck, and a forward-head posture that jams those joints all day. So the fix mirrors general neck rehab but aimed higher: mobilize and relax the upper neck, wake up the deep neck flexors (the chin-nod), and free the upper back and posture. As the neck calms and controls itself, the headaches fade.

Take your movement temperature. Frequent headaches, easily triggered by neck positions, tender stiff upper neck = Protection. Headaches settling, only with sustained postures = Controlled Motion. Occasional, ready for strengthening and endurance = Return to Function. The win is fewer, milder headaches that track with a calmer neck. Re-check every session.

The Rules That Govern This Protocol
